Instructions: Please follow the steps below to submit your policy recommendations. If the form is not completed correctly, recommendations will be returned to the county office of origin. The form must then be resubmitted.

Steps for policy recommendation submission:
1. Fill in your submission information. County, date, submitter name and submitter email.
2. Choose the number of policies you have to submit.
3. Indicate the type of policy you are submitting.
4. Enter the policy recommendation.
5. If the recommendation requires further explanation, enter that in the reasoning box.
6. After you have entered all recommendations, choose "Submit".

Submissions due to State Office by June 21.
